Sure, it’s big news when celebs tweet selfies from the dental office… if you’re still living in the 20th century. But in Hollywood today, it’s harder to say who hasn’t posted snaps of themselves in the dentist’s chair than who has. Yet the pictures recently uploaded to Twitter by Mark Salling, the actor and singer who regularly appears as Noah “Puck” Puckerman on the popular TV series Glee, made us sit up and take notice.

“Getting my chipped tooth fixed. Also, apparently, I’m a big grinder,” read the caption. The photo showed a set of upper front teeth with visible chips on the biting surface. What’s so special about this seemingly mundane tweet? It’s a great way of bringing attention to a relatively common, but often overlooked problem: teeth clenching and grinding, also called bruxism.

Although bruxism is a habit that affects scores of people, many don’t even realize they have it. That’s because the condition may only become active at night. When the teeth are unconsciously ground together, the forces they produce can wear down the enamel, cause chipping or damage to teeth or dental work (such as veneers or fillings), or even loosen a tooth! While it’s common in children under 11 years old, in adults it can be a cause for concern.

Sometimes, mouth pain, soreness and visible damage alert individuals to their grinding habits; other times, a dental professional will notice the evidence of bruxism during an exam or cleaning: tooth sensitivity and telltale wear and tear on the chewing surfaces. Either way, it’s time to act.

Bruxism is most often caused by stress, which can negatively impact the body in many ways. It may also result from bite problems, the overuse of stimulating substances (caffeine, alcohol, tobacco, and illegal drugs), and as a side effect of certain medications. Sometimes, simply becoming aware of the habit can help a person get it under control. Common methods of stress reduction include exercise, meditation, a warm bath or a quiet period before bedtime; these can be tried while we monitor the situation to see if the problem is going away.

If stress reduction alone doesn’t do the trick, several other methods can be effective. When bruxism is caused by a minor bite problem, we can sometimes do a minor “bite adjustment” in the office. This involves removing a tiny bit of enamel from an individual tooth that is out of position, bringing it in line with the others. If it’s a more serious malocclusion, orthodontic appliances or other procedures may be recommended.

When grinding is severe enough to damage teeth or dental work, we may also recommend a custom-made night guard (occlusal guard), which you put in your mouth at bedtime. Comfortable and secure, this appliance prevents your teeth from being damaged by contacting each other, and protects your jaw joints from stresses due to excessive grinding forces.

The big day you've waited for all your life is just around the corner — your wedding day! And to make that day as special as possible you've been working on making yourself more attractive.

In all your preparations, don't forget your smile. There are many ways to make it shine, some requiring little time or effort. A professional dental cleaning and polishing, for example, can do wonders for brightening your smile. If you have above normal staining, you can also undergo professional whitening to remove stains and enhance your teeth's natural color.

Some problems, though, like chipped, slightly misaligned or heavily stained teeth may require more than a cleaning or whitening session. In these cases, you might consider covering these less attractive teeth with porcelain veneers to transform their appearance. As the name implies, veneers are a thin layer of tooth-colored, translucent porcelain custom designed for you and bonded permanently to the visible tooth.

While veneers can significantly change your smile, it can't fix every appearance problem. Some teeth require more extensive dental work, like a porcelain crown that completely covers a tooth, or dental implants to replace missing teeth. In more complex situations you may want to look at orthodontics to repair an unattractive bite, or plastic surgery to change the look of a gummy smile.

Keep in mind, though, many of such treatments take time: installing dental implants can take months and some orthodontic treatments, years. As soon as you can, you should discuss your smile appearance with your dentist and what can be done to enhance it in the time you have.

With the help of your dentist, orthodontist or other specialist, you can change your smile. And that, along with all your other preparations, will help make that once in a lifetime day even more special.

Do you remember when you last flossed your teeth? This important oral hygiene task often gets neglected. Unfortunately, if you don't flossingfloss regularly, you'll increase your risk of tooth decay. Dr. John DiStefano in Glastonbury, CT, explains why you should add flossing to your daily to-do list.

Plaque takes over when you don't floss

Flossing doesn't just remove tiny bits of food from the spaces between your teeth but also gets rid of plaque. Plaque plays a pivotal role in tooth decay. If you don't remove the sticky coating from your teeth periodically, you'll be more likely to develop cavities. Although you might associate cavities with the chewing surfaces of your teeth, they can also form between teeth. Flossing offers a simple way to get rid of plaque and decreases your tooth decay risk.

Plaque isn't the only problem

When plaque first appears on your teeth, it can be easily removed simply by brushing and flossing your teeth. As time goes by, plaque eventually hardens and turns into tartar. Clear plaque can't be seen on your teeth, but brown or gray tartar can. No matter how much you brush your teeth, you won't be able to get rid of tartar. Unfortunately, if tartar develops at your gum line, it may cause gum disease.

Tartar build up can push your gums away from your teeth, creating spaces called pockets. When bacteria invade the pockets, an infection occurs that can cause your teeth to loosen and fall out. Flossing daily ensures that plaque never remains on your teeth long enough to turn into tartar. If you do develop tartar, a thorough dental cleaning at our Glastonbury office will remove it.

Make time for daily flossing

Flossing only takes a minute or two but offers important oral health benefits. If you make time for it once a day, you'll decrease plaque, prevent tartar build up and improve your breath by removing trapped food debris. When you floss, be sure to use gentle pressure as you move the floss between two teeth. Pressing too hard doesn't remove more plaque and can damage your gums. After you floss, rinse your mouth to remove loosened plaque particles.

Did you see the move Cast Away starring Tom Hanks? If so, you probably remember the scene where Hanks, stranded on a remote island, knocks out his own abscessed tooth — with an ice skate, no less — to stop the pain. Recently, Dear Doctor TV interviewed Gary Archer, the dental technician who created that special effect and many others.

“They wanted to have an abscess above the tooth with all sorts of gunk and pus and stuff coming out of it,” Archer explained. “I met with Tom and I took impressions [of his mouth] and we came up with this wonderful little piece. It just slipped over his own natural teeth.” The actor could flick it out with his lower tooth when the time was right during the scene. It ended up looking so real that, as Archer said, “it was not for the easily squeamish!”

That’s for sure. But neither is a real abscess, which is an infection that becomes sealed off beneath the gum line. An abscess may result from a trapped piece of food, uncontrolled periodontal (gum) disease, or even an infection deep inside a tooth that has spread to adjacent periodontal tissues. In any case, the condition can cause intense pain due to the pressure that builds up in the pus-filled sac. Prompt treatment is required to relieve the pain, keep the infection from spreading to other areas of the face (or even elsewhere in the body), and prevent tooth loss.

Treatment involves draining the abscess, which usually stops the pain immediately, and then controlling the infection and removing its cause. This may require antibiotics and any of several in-office dental procedures, including gum surgery, a root canal, or a tooth extraction. But if you do have a tooth that can’t be saved, we promise we won’t remove it with an ice skate!

The best way to prevent an abscess from forming in the first place is to practice conscientious oral hygiene. By brushing your teeth twice each day for two minutes, and flossing at least once a day, you will go a long way towards keeping harmful oral bacteria from thriving in your mouth.

It's a big transition when your child enters college — for both of you. You may find “cutting the apron strings” a little rocky at times.

But like most parents, you'll soon condense what you still want your college kid to do down to a few major habits and choices. Be sure to keep health, diet and lifestyle choices on that list, areas which could have the most effect on their long-term health and well-being.

That should include dental care. Hopefully, they've already developed good hygiene habits like daily brushing and flossing and regular dental visits. But, on their own now, they're faced with other choices that could affect their dental health.

For example, eating a balanced, nutritious diet is necessary for a healthy mouth. That includes limiting sugar intake, especially when snacking. Disease-causing oral bacteria thrive on carbohydrates like sugar. These bacteria also secrete acid, which at consistently high levels can erode tooth enamel.

Tobacco smoking and excessive alcohol affect teeth and gums because both can inhibit secretion of saliva. Besides containing antibodies that fight infection, saliva also neutralizes mouth acid. A dry mouth caused by these habits, could put their mouth at higher risk for disease.

Your college student might also be influenced by the fashion of their peers to display piercings. Mouth piercings with lip or tongue hardware in particular can damage teeth. The constant movement and friction erodes enamel or may even cause a tooth fracture. If possible, try to steer them to self-expression that poses less risk to their dental health.

There's one other area that, believe it or not, could impact dental health: sex. While each family handles this particular subject differently, be sure your child knows that some forms of sexual activity increase the risk for contracting the human papilloma virus (HPV16). Among its many destructive outcomes, HPV16 profoundly raises the risk of oral cancer, a rare but deadly disease with a poor survival rate.

Going from home to college is a big step for a young person — and their parents. As a parent, you can help steer them to practice good habits and make wise choices that will protect their lives and health and, in particular, their teeth and gums.
